Insurance & Appointments

We accept virtually all PPO insurance plans and have found that most insurance companies do cover the procedure. Dr. Mostowfi is in-network as a participating provider (PPO doctor) with most insurance carriers including Blue Cross/Blue Shield. His Blue Cross/Blue Shield provider number is 21625187 and his NPI number is 1700903309. We do not accept Tricare or any HMO plans.

So that we can help you estimate your out-of-pocket expense, please call the number on the back of your insurance card and:

  • Give them the Main Procedure Code for the Vasectomy (55250).  Make sure to let them know that the procedure is done “in-office” (not in the hospital) and ask if your policy has coverage for the vasectomy procedure.

  • Please also ask them the following 6 questions:
    1. What is my deductible for the year?
    2. How much of my deductible have I already met?
    3. What is my co-insurance percent? (ex: 90/10, 80/20, etc.)
    4. Am I responsible for a co-pay for either the Consultation Appointment or the Vasectomy Procedure Appointment?
    5. Is pre-certification required?
    6. May I have a Reference Number for this call?
Once you have the answers to the 6 questions above, please call us at 312-528-9068 (press option 1) and provide us with that information.  We will then be able to estimate your out-of-pocket expense for you.  Sometimes it is hard to know what to ask the insurance company so hopefully having a list of specific questions helps make it a little easier to get that information from your insurance carrier.   If you have any difficulty obtaining answers to these 6 questions, please give the insurance representative our telephone number (312-528-9068 option 1) and ask them to conference us in on your call so we can help you while you are still on the line with them.
